Document Builder Guide

Formatted Paragraph Text: Use this module to enter your waiver text. You can type of paste your waiver, format the text, add numbered lists, embed videos and more. 

Short Text Field: Use this module when asking a question.**

Large Text Box: Use this module when you would like your participant or customer to enter a long answer.**

Checkbox: Use this module when you would like your participants or customers to check and acknowledge something.**

Dropdown List: Use this module if you would like your participants or customers to select from a list, or use for Yes / No questions by deleting one of the default placeholders and making the choices Yes & No.**

Signature or Initials: Use this module if you would like your participants or customers to initial or sign in additional places in your waiver. All waivers have a preset final signature that cannot be removed, and this module allows for you to add more. 

Photo Upload: Use this module for ID or insurance document uploads. Images uploaded through this module are embedded into the waiver and also saved as an attachment for each contact.**

Space: Use this module to add space between modules or sections or adding a horizontal line.
